Contact Branches of Care to talk through your goals, ask a question or find out how our supports could work for you or someone you care for.
It’s our greatest joy to support every branch of every persons life.
We can’t wait to share time with you.
The information you provide will be used to respond to this enquiry and determine whether our supports may be suitable. Please see our Privacy Policy for information about how we collect, use and protect personal information.
When you contact Branches of Care, you’re taking the first step towards support that fits your life. With clinical nursing experience behind everything we do and a broad range of supports across home, skills and community, we can look at the whole picture and help you plan what comes next.
Whether you’re new to the NDIS, looking to change providers or supporting someone you love, we’ll listen, answer your questions and help you take the next step with confidence.
Here are answers to some common questions about getting started with us. If yours isn’t covered, send us a message and we’ll be happy to help.
Not necessarily. If you already have an NDIS plan, it helps to have it handy when we talk, as it shows us your funded supports and the goals you’re working towards.
If you’re still applying, waiting on a decision or getting ready for a plan review, you’re still welcome to reach out. We can talk through the kinds of support that might suit you and what to think about as your plan comes together, so you’re ready to get started once it’s in place.
We can support participants whose funding is self-managed or plan-managed. With these options, you have the flexibility to choose providers whose approach suits you.
Our NDIS registration is currently pending. Participants whose plans are NDIA-managed (sometimes called agency-managed) can generally only use registered providers, so we’re not able to support NDIA-managed funding just yet. If your plan is NDIA-managed and you’d like to work with us, it may be worth asking your Support Coordinator or Local Area Coordinator whether plan management could be an option at your next review.
After you contact Branches of Care, someone from our team will be in touch to arrange a time to talk. This first conversation is all about getting to know you: your goals, your routine, what’s working in your current support and what you’d like to change.
From there, we’ll talk through which of our supports could help and answer any questions you or your family have. If it feels like a good fit, we’ll work with you to put a plan in place that reflects your preferences and choices.
You’re welcome to take your time deciding, and to bring along anyone you’d like to be part of the conversation.
Absolutely, if that’s what you’d like. Many participants feel more comfortable having a family member, carer or trusted friend involved, whether that’s joining the first conversation, helping to set goals or staying updated along the way.
You decide who’s involved and how much. We’ll always respect your choices about your information and your support.
Needs change, and your support should change with them. Whether your health shifts, you reach a goal or you’re ready to take on something new, we’ll review your support with you and adjust it to suit where you’re at.
Because our founder is a registered nurse with experience in complex and high intensity care, we’re well placed to keep supporting participants as their health needs become more complex. And because we offer supports across home, capacity building and community access, adding a new area of support can be as simple as a conversation.
If you notice a change, contact Branches of Care and we’ll talk through what would help.
Yes. Some participants use more than one provider, for example keeping a provider they’re happy with for one type of support while trying us for another. You’re in control of your plan and your choices, and we’re happy to work alongside other providers so your support fits together smoothly.
If you’re thinking about moving all of your support to us, we can talk that through too.
