When you send NDIS referrals our way, you’re connecting participants with a provider led by a registered nurse with hospital, specialist clinics, and disability care experience. That means we can confidently support participants with complex and high intensity needs, including tracheostomy management, ventilation care and PEG feeding.
We also offer supports across daily living at home, capacity building and community access, so participants can work towards several goals with one provider, making life simpler for everyone involved.
Referring someone is a matter of trust. We work closely with Support Coordinators, Plan Managers and families, keeping communication open and honest so everyone knows how the participant is progressing towards their goals.
Our focus always stays on what the participant wants to achieve, and we’ll share their progress with you along the way. Whether you’re making your first referral or looking for a new provider for someone you support, we’d love to hear from you.
